What the two labels mean, what the only large randomised trial found, and a plain verdict by sleep position, body weight and back pain, with the caveats the product pages leave out.

Medium-firm or firm: which should you choose?
Quick answer: Medium-firm suits more people than firm. It gives enough support to keep the hips level while letting the shoulders and hips sink slightly, which is what most back and side sleepers need. Firm suits stomach sleepers, heavier sleepers, and people who prefer a flat, lifted feel. The best-known trial, published in The Lancet in 2003, found adults with chronic low back pain did better on medium-firm than firm.
Choose firm when your hips sink on a medium-firm bed, not because you think firmer is healthier. What "medium-firm" and "firm" actually mean
There is no industry standard for the 1 to 10 firmness scale. Each brand assigns its own number, so a 7 from one company can feel like a 6 or an 8 from another. Broadly, medium-firm sits around 6 to 7 on most brand scales and firm around 7.5 to 9. The label describes how far the surface gives under you before the support core takes over, not how supportive the bed is. A soft comfort layer over a strong coil unit can be plush at the surface and supportive underneath.
Research uses a different scale entirely. The Lancet trial rated mattresses on a European scale where 1.0 is the firmest and 10.0 the softest; its firm bed was 2.3 and its medium-firm bed 5.6. Converting that to a brand's 1 to 10 is not straightforward, so treat any page that claims "science says a 6.5" with suspicion.
What the research supports
- The Lancet, 2003. 313 adults with chronic non-specific low back pain, randomised to a firm or medium-firm mattress for 90 days. The medium-firm group reported less pain lying in bed and less disability. The trial did not include soft mattresses, side-sleeping analysis or sciatica.
- Sleep Health, 2015. A systematic review of 24 controlled trials found medium-firm and self-adjusted mattresses best for comfort, sleep quality and alignment, and noted that firmness was defined inconsistently across studies.
- CADTH, 2022. A Canadian rapid review on therapeutic mattresses for chronic pain found medium-firm outperformed firm in the studies reviewed, and concluded that more robust research is needed.
The honest reading: medium-firm is the evidence-backed starting point for people with back pain, the evidence is limited, and nothing in it says a firm bed is harmful for someone who sleeps well on one. The two mistakes people make
Buying firm to fix back pain. The trial evidence points the other way for most people. If your pain is worse on waking and eases as you move, you are more likely to be too firm than too soft.
Buying medium-firm and then adding a thick topper. A 3-inch plush topper turns a medium-firm bed into a soft one, and the support you paid for stops reaching your body. How to decide without guessing
- Weigh yourself honestly and name your main position. Our firmness chart by position and body weight gives ranges.
- Lie on your back on the candidate bed and slide a hand under your lower back. A hand that fits easily is the neutral curve. A forearm means too firm. A pinned hand means too soft.
- If you share the bed, choose for the lighter sleeper's pressure and the heavier sleeper's support. Medium-firm is usually where those meet.

Is a medium-firm or firm mattress better for back pain?
Quick answer: Medium-firm, on the available evidence. A 2003 randomised trial in The Lancet found adults with chronic low back pain had less pain and disability on a medium-firm mattress than a firm one over 90 days, and two later reviews agreed. The evidence is limited and does not cover every kind of back pain, so persistent pain needs a clinician, not a mattress.
